What are Intellectual Property Rights? IPR are rights given to creators for them to have exclusivity over their creation for a specified period of time. In Korea, please visit the Korean Intellectual Property Office KIPO for the IPR registration process. · Exclusive sales rights, distribution contract violation and low selling prices are matters agreed upon through an individual contract, so they cannot be quoted as a reason for reporting an intellectual property infringement. 1) Document to prove the ownership of right(s)

Choose one of the following : Certificate of Business Registration or Photo of the rights holder's ID * You can submit additional documents such as Trademark License Agreement, etc. * A Certificate of Business Registration issued within 3 months of the report submission date (=Certificate of Business Registration Statement). * Rights holder's ID : Masking is required for personal information, except for the name and the date of birth & ID card photographed against a background that shows both the date and current time (hour/minute/second) (However, adjustable personal watches are not acceptable)

2-1) Attachment of documents that can prove the right(s)

For the registration certificate of right(s), copy of a court judgment, and copy of a decision by an administrative agency, Please check the notes in the table below and submit the documentary evidence such as a screenshot of the copyright registration certificate, or work materials that is able to verify the original file and job layers (PhotoShop or equivalent work programs).

2-2) Attachment of documents that can prove rights(s)

*Important : If the registration status of the rights is not "Registered" but [Application Filed, Application Published], it means that the trademark rights have only been requested from the Patent Office. Therefore, direct actions such as suspending the sale of the reported party's products are difficult during the Filed and Published stages.

3) Document to prove the infringement of the right(s)(optional)

A copy of the court judgement, or a copy of a decision by an administrative agency, etc.

4) Letter of delegation and identification of agent(optional)

If the reporter is an agent, the agent’s identification is required. And it is allowed to submit the agent’s certificate of employment instead of a copy of identification.

* The power of attorney : the names of the delegator and the representative, the content stating the delegation of intellectual property rights infringement tasks, and the delegation period (include the submission date or indicate as 'No period restriction' ) * Certification of Employment: Issued within the last 6 months from the reporting date. * Representative's ID : Masking is required for personal information, except for the name and the date of birth & ID photo taken with the current date and time clearly visible in the background, as shown when searching "current time" on an internet portal site. (The photo must be taken within 1 week of the report submission date.) <strong><Example of identification with personal information blotted out></strong> Please submit a copy of identification with the information blotted out other than the date of birth and gender for personal information protection. 1) Please attach a copy of identification with your personal information blotted out (last 7 digits of resident registration number, photo, address, date of issuance, date of issue and expiry, etc.) other than your name and date of birth. 2) Please make sure to blot out the unnecessary personal information when you attach a copy of identification.

**A copy of identification submitted without the information blotted out will be destroyed immediately.

I (or Agent) pledge as follows: 1) Information and evidential documentation provided for reporting an intellectual property infringement are accurate without any falsification and I will take full responsibility for legal issues arising from any false information and any damage to the company, the infringer, and the third parties as a result of them. 2) When reporting a copyright infringement, I have confirmed that measures can be taken against the infringer’s product based on my request that explains I (or Agent) am the creator of the work, and if there is any dispute involving the report in the future, I agree to inform the infringer that it has been proceeded at my (or Agent's) request. 3) I will not report any infringement of intellectual property right(s) beyond the scope of my own right(s) or which may disrupt the infringer's or your company's fair business activities. If an infringement report of mine causes any damage or civil/criminal obligations, I (or Agent) will assume all the obligations to exempt your company from them. 4) I (or Agent) agree to the transfer of this infringement report form along with my personal information (or Agent’s) to the infringer for the purpose of reporting an intellectual property infringement and I will resolve any legal dispute arising from this report directly with the infringer.
